Boruto Uzumaki’s transformation into an Ōtsutsuki is one of the most important developments in Boruto: Naruto Next Generations. After Momoshiki implanted the Kāma into Boruto, the seal gradually converted his body into Ōtsutsuki genetic data. This meant that Boruto was slowly becoming a vessel capable of allowing Momoshiki to resurrect through him. As the Kāma progressed, Boruto also became increasingly vulnerable to Momoshiki taking control of his consciousness.

The situation became even more dangerous after Boruto and Kawaki’s Kāma marks resonated with each other. Boruto eventually reached the point where his transformation could no longer simply be stopped through willpower. Amado therefore offered a temporary solution in the form of medication designed to suppress the progression, although the treatment carried potentially dangerous side effects.

The Kāma functions as a compressed backup of an Ōtsutsuki’s biological data. As Momoshiki’s Kāma continued extracting information into Boruto’s body, Boruto’s physical composition increasingly became Ōtsutsuki. This process eventually reached completion after Momoshiki used the remaining Kāma data to repair the damage to Boruto’s body and revive him following Kawaki’s attack.

This resurrection had a major consequence. Boruto became a complete Ōtsutsuki, but Momoshiki could no longer use him as a conventional resurrection vessel because the remaining data had already been consumed to restore Boruto’s body. Momoshiki’s consciousness nevertheless remained inside Boruto, meaning the transformation did not eliminate the threat.

Amado’s pills were created for a different purpose before being given to Boruto. They were designed to weaken the Byakugan, and Amado believed suppressing that Ōtsutsuki related ability could also slow Boruto’s transformation. The medication temporarily prevented the Kāma from progressing, allowing Boruto to use its power while maintaining control, but it was never presented as a permanent cure.

The treatment also carried potential side effects because Boruto’s circumstances differed from Kawaki’s. Amado had previously used the drug on Kawaki in smaller doses, but Boruto’s connection to the Hyūga bloodline meant the effects could be different. Boruto nevertheless chose to take the pills because he was determined to prevent Momoshiki from taking control.

Ultimately, Amado’s pills could only delay Boruto’s Ōtsutsuki transformation. Momoshiki’s decision to use the remaining Kāma data to revive Boruto completed the transformation, permanently making Boruto an Ōtsutsuki while also changing the nature of his connection with Momoshiki.
